  3. Then select the Selections button.

  5. Select the Add New button, to choose which registration status to search on.
  6. Choose Applied and scroll down the list to select Permanent.
The criteria updates on the left hand side as you add items.

  7. Select OK to add.
    The main search details update.
  8. Select the Add Entity button next to Search Details.
The Select from Group screen displays.
  9. We want to search for a medical history item, so double click on All other Clinical Data, and again on Medical and Family History folder.

  10. Select Medical History and click OK.
    The Search Details refresh.
  11. In the Search Details box, select Medical History and then select the Selections button.

If you leave the tick in the Inclusive box this includes today.

  14. Select Add New, to add more criteria.
  15. Scroll down until you see the Read code for general entity option, and select.

Note - If you leave the Hierarchy and Synonyms box ticked the search returns any matches below this entry in the hierarchy.
  18. Select OK to update the Search Details.
  19. Select Run to generate the report.
  20. Optionally, select Save As to save the report for future use.
